This submission form is to report any imports and/or exports of controlled substances under the Ozone Layer Protection Act 1996 throughout each calendar year.
[b]This information is to be submitted to the EPA [u]no later than 31 January[/u]. [/b]

[/b]This reporting requirement is set out in conditions applied to permits/exemptions in accordance with the Ozone Layer Protection Act 1996 (OLPA) and the Ozone Layer Protection Regulations 1996 (OLPR). Failure to comply with this request may result in compliance action.

[/b]· [b]Controlled Substance [/b]means any substance specified in Schedule 1 of OLPR;
· [b]New Substance[/b] relates to new manufactured controlled substances;
· [b]Bulk [/b]means[b] [/b]any controlled substance that is acquired in a non-processed form, whether alone or in a mixture. It [u]excludes[/u] any controlled substance that is in a manufactured product other than a container used for the transportation or storage of the substance
· [b]Bulk recycled substance[/b] means any controlled substance that is acquired in a non-processed form, whether alone or in a mixture, and that has been recovered, cleaned, or reclaimed; but excludes any controlled substance that is in a manufactured product other than a container used for the transportation or storage of the substance
· [b]New bulk HFC [/b]means a bulk hydrofluorocarbon (HFC) that is not a bulk recycled substance
· [b]Import/Export Entry Numbers[/b] relates to numbers issued by New Zealand Customs for each import/export
Only the amounts of controlled substances imported, transhipped, and exported during the previous calendar year are to be reported in the form, not any additional stock/stores that your company may be holding.
This information is required for international reporting and to assess compliance with your permit. Therefore, it must be accurate, and reflect your own company records. Information reported may be made publicly available, including on the EPA website.
